There is a list of dental procedures with a co-pay amount after each procedure. This is how much you pay.

Preventive procedures like cleanings have a $0 co-pay.

If you need to go to a specialist, your fee will be 75% of their usual and customary fees.

Two Different Plans

You can choose the Premier Choice or the Classic Choice. One has slightly higher co-pays than the other.

How To Apply

You can open and print an application or save it to your hard drive and print it out later. The application must be mailed in according to the instruction sheet in the application packet.
